Employee Leave Requests

  • Bereavement Leave - Please complete Bereavement Leave request form.  Enter absence in Absence Management, select reason as Bereavement Leave or OFLA Bereavement (form required).  In the notes to the administrator, note the date you were notified of your eligible family member's death as well as the relationship of the immediate family member* to you. 
​                  *Immediate family members are defined as an employee's mother, father, brother, sister, child, step-child, mother-in-law,
                    father-in- law, brother-in-law, sister-in-law, those who stood in loco parentis, spouse, spouse's and employee's grandparents,                              grandchildren, a person regularly residing in the household and domestic partner.  Relations not included in this list are             
                    ineligible for employee use of paid and/or OFLA Bereavement leave

  • Please review the information regarding the provisions of Bereavement Leave in the Collective Bargaining Agreement for your association. (NWEA - Article 8.B or OSEA - Article 13.2) ​  
  • Jury Duty - Enter absence in Absence Management, select reason as Jury Duty, provide Human Resources with a copy of your Jury Summons (email, courier, or fax 503-614-3186).  You have the option of declining payment from the court, or any monies paid to you (excluding mileage which is yours to keep) from the court may be signed over to payroll or via personal check made out to "NWRESD" with "Jury Duty" written in the notes.
 
  • Personal Leave - Enter absence in Absence Management, select reason as Personal Leave, follow up immediately with an email to your supervisor to request authorization for Personal Leave.  Your supervisor will then reply to your email with an "approval".  You will be contacted by Human Resources if your request was "denied".
 
  • Unpaid leave request - Fill out the required google form by clicking on the hyperlink. If you have any questions or need support, please email kfernandez@nwresd.k12.or.us ​
