Equipment loss
Please follow the steps below if your Northwest Regional ESD issued equipment has been lost or stolen.
Within the first four hours of loss
By the end of the first working day
4. Provide this data inventory to your supervisor.
By the end of five days
Within the first four hours of loss
- Notify your supervisor of loss.
- If stolen, report loss to the police department.
- Email support@cascadetech.org, or call 503-614-1400 to submit a ticket. Please include a list of equipment and description of the situation around the loss.
By the end of the first working day
- The support desk will provide you with identifying equipment information including make, model and serial number. Write down this identifying information.
- If stolen, contact the police department to update them with the identifying information.
- Complete an inventory of all data stored on or with the equipment. This may include:
- IEP or IFSP
- Spreadsheets and documents containing personal identifiable information*
- Student case files
- USB flash drives, CD’s, external hard drive
4. Provide this data inventory to your supervisor.
By the end of five days
- If stolen, obtain copies of the police report and email to support@cascadetech.org
Equipment return
Equipment provided to you by the technology department does not need to be turned in for the summer unless you are leaving employment. If you are leaving employment, please print and complete the Equipment drop-off form, and turn it in with all assigned technology equipment. Drop off equipment at the receptionist desk at the service center closest to you (Clatsop, Columbia, Tillamook or Washington).
Please check with your supervisor for instructions regarding other types of equipment managed by your department, such as iPads. This equipment might be collected over the summer, even if you are not leaving employment.
